With the 2007 VSL Mets, he allowed four runs in 12 innings over 5 games. He had a 2-0, 6.00 record in 11 outings for the same team a year later. After a year off, he was 1-1 for the 2010 VSL Reds but gave up 13 hits and 12 runs (9 earned) in 4 2/3 IP. Overall, he had gone 3-1 with a 6.49 ERA in 20 minor league games. In 34 2/3 IP, he had allowed 39 hits, 20 walks, 10 hit batsmen and 11 wild pitches while striking out 16.

Josmar was on the Venezuelan national team in 2011. In the 2011 Baseball World Cup, he was their second-worst hurler (only George Delgado was worse). He turned in a strong opening against the Dominican national team (2 BB, 2 H, 0 R in 5 IP) but faded in the 6th, walking Bernie Castro and Alexis Gómez with a single by Willis Otáñez in between. Jorge Guzman relieved and let all three runs score; Venezuela and Carreño got the win. In his second game, he was toasted by Team Canada for 7 runs and 6 hits in 1 1/3 IP before being yanked in favor of Luis Azócar. Among the hits were a two-run double to Emerson Frostad and a two-run homer to Brock Kjeldgaard. In the 2011 Pan American Games, he was better, with a 1 1/3 innings, one hit, one walk, one strikeout and one unearned run. He was 1-0 with only 3 hits, 3 walks and 2 runs in 8 2/3 IP in the 2013 Bolivarian Games as Venezuela won the Silver. He got the win over Guatemala in the semifinals, with two perfect innings of relief.

He was Venezuela's top hurler in the 2014 Central American and Caribbean Games, with no runs in 9 2/3 IP. He allowed six hits and walked none while fanning nine, with a 3-inning save to team with Jhonny Caraballo in a shutout win over Nicaragua (which finished second). He got his win over Guatemala. He led the event in ERA and tied Luis Cintron for 6th in whiffs. He got into one game in the 2015 Premier 12, relieving former major leaguer Carlos Monasterios with a 3-0 deficit in the first against eventual champion South Korea. He was their top hurler of the day, throwing 1 1/3 shutout innings (1 BB, 1 K) before turning it over to Robert Palencia with a 3-2 game in place; Venezuela would lose, 13-2.

In the 2017 Bolivarian Games, he allowed one hit and one walk in 3 1/3 shutout innings; only Carlos Parra and Juan Colmenarez pitched more shutout innings for Venezuela.
