Juan Serrano has been a pitcher in the Nicaraguan amateur league, where he set the save record with 18 one year.

Serrano pitched for the Nicaraguan national team in the 2011 Baseball World Cup. In four games, he went 0-1 with a 5.68 ERA. Despite lasting only 6 1/3 IP, he tied for the Cup lead with four hit batsmen. He pitched shutout relief against Italy and Venezuela, but surrendered four runs (three earned) in two innings against the Dominicans. Against Australia, he took the loss, relieving Esteban Pérez with a 2-2 score, the bases loaded and no out in the 5th. He allowed a sacrifice fly to Tim Kennelly then gave up the final run of a 4-3 loss when Brad Harman and James Beresford both doubled in the 6th.
