Only three times has there been a man on first, and there was in intentional walk anyway (twice first and third, once just first). All three times, the walker was Barry Bonds.

A lot of the Bonds intentional walks started as attempts to get him out on the edges, having no chance of getting a borderline call on a good pitch, running it to 2-0 or 3-0, and then just giving up and walking him. I'd be curious how many of these IBB were "four wide ones", and how many were "the count favours him, so just give up". I assume the IBBs with runners on first were like this, but maybe not.

I don't think it's happened in the postseason, but there have been intentional walks with the bases loaded, perhaps with Babe Ruth at bat.

Buck Schowalter had the DBacks do it to Barry Bonds in a game in 1998.

I have read of three occasions of intentional walks with the bases loaded, all not in post-season. Once to Nap LaJoie, once to Mel Ott and once to Phil Cavaretta. The one to Mel Ott was not out of respect. In their last game of the 1929 season Phillies pitchers walked Ott on purpose 5 times so as to deny him a chance to overtake Chuck Klein for the HR title. One of those at-bats was with the bases loaded.

It's been discussed here at B-R before, I don't recall all the details but Ruth was not one of the names.
This is from the Baseball Almanac, research by Bill Deane and Everett Parker:
"Abner Dalrymple, August 2, 1881 - the Bisons were were down 5-0 versus the White Stockings in the eighth inning. The "Chicago Tribune" described the moment with, "In the eighth the bases were filled, and nobody out, on successive hits by (Fred) Goldsmith, (Silver) Flint, and (Joe) Quest, and (Jack) Lynch was so afraid of Dalrymple that he gave him his base on balls and brought Goldsmith in with the gift." Another Tribune quote which described the moment said, "At one time, when the bases were full, Lynch deliberately sent in seven balls rather than take the chances of a hit by Dalrymple, who was at bat, and in this way forced a run upon Chicago."
Napoleon Lajoie, May 23, 1901 - the Athletics were batting against the White Sox in the top of the ninth inning. They were down, 11-7, but had the bases loaded and none out with Lajoie - on his way to a Triple Crown - up. Manager Clark Griffith inserted himself as relief pitcher and "calmly sent four wide ones across" to deliberately force in a run, bringing future home run champs Socks Seybold and Harry Davis up with the bases still full. Griffith then disposed of Seybold, Davis, and Morgan Murphy on infield grounders, saving the 11-9 win.
Del Bissonette, May 2, 1928 - the Giants led the Dodgers, 2-0, in the top of the ninth, but Brooklyn loaded the bases with two out. Giants' manager John McGraw then ordered pitcher Larry Benton to walk Bissonette, in his 16th major league game, purposely ending the shutout. Benton then struck out Harry Riconda to end the game.
Bill Nicholson, July 23, 1944 (2nd game) - The Giants led the Cubs, 10-7, in the top of the eighth with the bases loaded, none out, and Nicholson - who had homered four times already that day - at the plate. Giants' manager Mel Ott (who was probably sitting next to McGraw when he ordered it done on May 2, 1928) ordered Ewald Pyle to walk Nicholson, bringing up Dom Dallessandro, Andy Pafko, and Don Johnson. The Cubs tied the game up by the time the inning was over, but New York went on to win, 12-10.
Barry Bonds, May 28, 1998 - The Diamondbacks led the Giants, 8-6 in the bottom of the ninth, with two out, the bases loaded, and Barry Bonds up. Arizona manager Buck Showalter ordered Gregg Olson to issue the IBB, putting the tying run on third and the winning run on second. Brent Mayne then lined out to end the game.
Josh Hamilton, August 17, 2008 - Bottom of the ninth, Rangers trailing by five with the bottom third of their batting order due up. Six batters later, Texas had scored one, recorded two outs, but had the bases loaded with Josh Hamilton stepping to the plate. Tampa Bay Rays' manager Joe Maddon told Grant Balfour to intentionally walk Hamilton, not only forcing in a run, but bringing the winning run, Marlon Byrd, to the plate"

It was Bill Nicholson, not Cavaretta who got the intentional walk as stated in post 10. I frequently get them confused.
Mel Ott's intentional walk was on this date in 1929. You can get details in the Today in Baseball History column

@9, @11/ Richard Chester - also, Klein had broken Roger Hornsby's NL-record of 42 HR, set in 1922.
I didn't realize one of those walks to Ott was with the bases loaded. I wonder if that broke an "Unwritten Rule", or if that (all the IBB) was controversial at the time?

I'm not sure if this has ever happened, but I suppose it could happen that the pitcher begins to intentionally walk the batter with bases loaded but there's a wild pitch or passed ball and runners advance. The intentional walk ends up happening with runners on second and third, even though the decision to issue the IBB was made when the bases were loaded.
There the similar case that's probably a lot more common - with bases loaded you throw a wild pitch/passed ball, then decide to issue an IBB since first base is now open. I'm guessing that's what happened for the IBB to Frank Robinson in that list.
Is there any way to find out more information about the IBBs where pitcher had gotten some strikes in the count? The IBB to Jim Edmonds came after David Eckstein stole second, but I didn't see any other cases where the runners advanced during the at bat (I'm sure it happened more, I was only looking at counts with strikes and that's a short list because the data isn't complete). Did the batter swing at one of the intentional balls? Did the pitcher try pitching to the batter, give up a long fly foul ball, then decide to walk them?

If you click on the date of the Frank Robinson game you retrieve the box score and the play-by-play. It does show a passed ball with the bases loaded and a subsequent IBB for Robinson.

There are occasions, when a base is open, that the decision to walk the batter will come when the count reaches 3 balls. The intentional walk occurs on the last pitch regardless of whether or not the other pitches were intentionally thrown out of the strike zone or not.

@9,11,12. Fitting w/ Hornsby. 1929 was his last great year and actually slugged higher than both Ott and Klein. He lead in slugging and OPS/OPS+ and had 400+ TB!

This isnt really about this subject but I remember watching a Jays game when John Olerud was tearing the league up, we was facing Denis Martinez, their was a runner on second with the jays down by 1 run. With firstbase open they pitched to Olerud. The first 2 pitches were right on the corners to make it 0-2. Martinez through 3 straight balls and then the catcher stood up and signaled for an intentional ball 4, Martinez came set at fired a fastball right down the pipe as the catcher sat down. Strike 3! Havent seen it happen since. Brilliant and risky

The Reds did that in a '70s World Series game vs the A's, I think. Never heard about it happening to Olerud -- on a 3-2 count, no less? I need to do a Tracer search on this one.

Martinez only struck out Olerud once. It was on 7/30/96. There was a runner on second, and the count was 3-2. Toronto was actually up by 2, however. And Olerud was not having an especially good season, batting 6th. It's hard to believe CLE would pretend to IBB in that situation.
BUT....it looks like you're right! The full articles aren't readable, but the blurbs show that it did happen.
